True gynecomastia is caused by an increase in the size of male breast tissue which is a fibrous, rubbery structure (different to fat). Therefore, doing frequent exercise or losing weight does not solve the problem in the majority of cases.

 

Pseudo-gynecomastia is a relatively common condition where males possess excess subcutaneous body fat in the chest region. There are no enlarged breast glands as such. However, this condition is often associated with being overweight.

 

Gynecomastia can be attributed to several causes. The hormonal imbalance between the hormones testosterone and oestrogen can make breast tissue grow in some individuals more than others. This phenomena usually occurs during puberty.

The increase in the size and amount of fat cells in the chest area happens in males as they reach middle age. This is due to a slowing in the metabolism and decrease in testosterone production.

One of the most common causes of gynecomastia we see here at Medisculpt™ clinic is the use of performance-enhancing drugs, especially anabolic steroids.

Other rare causes of gynecomastia may include:

  • Alcohol abuse (which increases the risk of enlarged male breasts)
  • Liver disease and Kidney failure
  • Radiation treatment of the testicles
  • Klinefelter’s syndrome
  • Infections or lumps in the testicles
  • Thyroid gland disorders
  • Medications (several types can cause the breasts in men to enlarge, including:
  1. Certain antibiotics
  2. Certain ulcer medications
  3. Chemotherapy
  4. Tricyclic antidepressants
  5. Diazepam (Valium) and some other drugs for treating anxiety
  6. Some HIV medications, including Efavirenz (Sustiva)
  7. Anti-androgens, commonly prescribed for patients with cancer or prostate enlargement
  8. Some heart drugs, including calcium channel blockers and Digoxin.

 

The signs and symptoms of gynecomastia may occur in one or both breasts. This may include:

  • Swelling of breast gland tissue 
  • The breast gland becoming tender
  • The increased diameter of the areola or if the chest tissue becomes asymmetrical. (The areola is the ring of pigmented skin surrounding the nipple.)

Note: You are advised to see a doctor if there is unusual and persistent swelling, tenderness, pain, and/or nipple discharge.

 

Gynecomastia is usually diagnosed by examining the patient's history and conducting a physical examination, accompanied by radiological (ultrasound) investigations when required.

On average, this procedure can take up to 120 minutes.

 

At Medisculpt, we routinely perform gynecomastia surgery as a day procedure without the need for hospital admission. The process involves removal of fat (Vaser Liposuction) and the excision of glands using the minimally invasive surgical technique.

Note: Liposuction is only performed if the chest wall has an increased amount of fatty tissue around the gland.

 

We routinely perform this procedure under safe doses of local anaesthesia while the patient is conscious. 

We also offer this procedure under Twilight Sedation at an accredited day hospital in Subiaco, in case the patient prefers to be asleep during the surgery. (Additional costs are associated with this option.)

 

Once the treatment area has been numbed, you will be unable to feel any pain. At Medisculpt™ we make every effort to ensure that every procedure we perform entails minimal pain or discomfort.

 

Most people tolerate this procedure exceptionally well. Patients can go home straight after the surgery and do not require prolonged post-surgical hospital admission or monitoring.

 

In the majority of cases, the incision scars are barely noticeable and begin to fade quickly within a few weeks.

At Medisculpt, we have the most extensive range of Fractional CO2 Lasers to help reduce any scarring, whether related to a surgery or trauma.

Patients may experience some numbness in the treated area after the surgery. However, this effect will disappear with time.

 

Most patients can return to office-based work within 5 to 10 days. We recommend at least 2 to 4 weeks off if your work involves heavy physical activity or travellingA Medical Certificate can be organised, which will not mention the name of your procedure.

 

Light activity is suitable after 72 hours, while light exercise is usually acceptable after two weeks. It is critical not to use the chest muscles for any physical activity for at least 2 weeks after the surgery.

 

Like any surgical procedure, this procedure comes with possible risks and complications that include, but are not limited to:

  • Infection
  • Over-correction
  • Inadequate correction
  • Delayed healing and unfavourable scarring
  • Nerve irritation and numbness
  • Temporary skin discolouration in the incision lines

 

  • These risks and complications may include:

  • Respiratory depression and compromise
  • Cardiovascular depression and compromise
  • Increased risk of Deep Vein Thrombosis (DVT)
  • Prolonged nausea and vomiting
  • Extended hangover
  • Short-term and long-term amnesia (memory loss) are some of the significant side effects.

 

In any case, all surgical procedures carried out under General Anaesthesia or Twilight Sedation should only be performed in an accredited hospital under the supervision of a qualified anaesthetist.

 

Very high quantities of local anesthesia are known to cause systemic toxicity which can result in cardiovascular compromise. 

At Medisculpt, all procedures carried out under local anaesthesia are done using a minimal amount of local anaesthetic agent to ensure patient safety.

 

To be considered suitable for undergoing any elective cosmetic procedure, you have to:

  • Be physically fit and active
  • Have a balanced and healthy diet
  • Be of sound emotional/mental health
  • Have realistic expectations of the outcome 

 

People unfit to undergo this procedure are:

  • Physically unwell with pre-existing medical conditions
  • Those with a history of keloid scarring
  • Those with unrealistic expectations
  • Those who are going through emotional stress and trauma
  • Those we are unable or unwilling to follow a healthy lifestyle (regarding their exercise and diet) after the surgery

 

We recommend that you do all your grocery shopping before surgery and restock your fridge and pantry with healthy food. We also recommend that you organise for someone to give you a lift back home as driving is not recommended for the first few days after the surgery. You should also consider arranging for a family member or friend to stay with you for the first few days after your surgery.

Additionally, ensure that you avoid blood-thinning medication such as Nurofen, Aspirin, Heparin and Warfarin. Please discuss this with your General Practitioner in detail and make sure you are aware of any side effects of your current medication.

Herbal medication and nutritional supplements such as St John’s Wart, garlic, ginger, ginseng, fish oil, and homeopathic or herbal tonics should also be ceased for at least two weeks before your surgery. These substances are known for their potential to thin blood and as such, may cause excessive bleeding during and after the surgery.

 

It is imperative that you follow the pre-surgical instructions given to you by our staff. Alcohol consumption and cigarette smoking are two primary culprits when it comes to increasing potential risks associated with any surgery. Please avoid these harmful agents for at least two weeks (or more) before your surgery.

Consumption of alcohol and cigarettes:

  • Interfere with the body's natural healing processes
  • Increase the pain and discomfort associated with any operation
  • Increase the risk of potential complications such as bleeding, wound infection and scarring
  • Prolong the recovery time.

 

In clinical practice, we have found that a herbal substance called Arnica is helpful in reducing bleeding during or after the surgery. It also aids healing and minimises bruising. This naturally occurring herb is available in the form of oral supplements and is found in most health food stores and pharmacies in Australia.

Above all, a healthy lifestyle, good sleep and a balanced nutritional diet after surgery are essential factors for optimum healing.

 

A cooling-off period of 2 weeks is required after your initial consultation to give you ample time to thoroughly evaluate the risks and benefits of your chosen elective cosmetic surgery.
